How to Register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

You can register by completing the online registration form. Please click on this link.

If you are completing the registration form with the above link, please also complete our Data Sharing Form (age appropriate) and Shared Administration form. Please be aware that these two forms need to be completed prior to registration.

14 years old and OVER_

UNDER the age of 14 years old

If you are unable to complete the registration process online, registration forms are available to collect from reception or you can download the forms below. You can complete and email the forms to us at boxsurgery@nhs.net or hand into reception.

For patients 14 years old and OVER

Please complete the NHS GMS1 form, the Box Surgery Registration Form, the appropriate Data Sharing Consent Form and the Shared Administration Consent Form.

For patients UNDER the age of 14 years old

Please complete the NHS GMS1 form, the Data Sharing Consent Form and the Shared Administration Consent Form.

You can hand the completed forms into Reception, post them to us or send via email.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Organ & Blood Donation

After 30 September 2021, your GP surgery can no longer record your organ and blood donation preference. If you wish to record your preferences, you can still do so directly through the blood and organ donation online registration websites, or by phone:
