Patient Triage 

Patient Triage has been designed to ensure that patients are booked in the most appropriate clinic, and at the right time, according to their clinical needs.

This is a great way for you to contact our practice for medical or admin requests for yourself or someone you care for. It is very simple to fill out, so please give it a go. Please try to give as much specific and concise information as possible. We think it will be easier for most patients to use, rather than waiting on hold to speak to a receptionist, but we would love to hear your feedback.

We understand that change can be difficult, but we believe that this new approach will ensure that patients’ needs are met safely and efficiently.

There is now the facility for patients to order repeat prescriptions via this website. This is protected by the highest security and cannot be accessed by others providing you keep your registration details secure. You can also gain access to a summary and coded information from your medical record.

NHS App

You can use the NHS App to order your repeat medication, view your coded medical record, view test results and, in the near future, use our Patient Triage system.

Request a Repeat Prescription

Please allow a minimum of 2 working days from the time of your request.

You can only request repeat medication that you regularly take. Your prescription will be sent to your usual pharmacy (as noted on your records). If you wish to change your usual pharmacy, please let us know.

View a Summary of your Medical Record

You can view your medication, allergies and adverse reactions.

View Coded information from your Medical Record

You can view diagnoses, pathology results, vaccinations, blood pressure readings, etc

Register to use SystmOnline

You will need to obtain a user name and password.

To obtain this please call at reception and complete an application form. You must bring some photo identification with you (such as a passport, driving licence or student card) as we will need to verify your identity. We will not be able to give you your letter without this ID verification.

You must keep your personal details secret and take all reasonable precautions to prevent the fraudulent use of your personal details. We reserve the right to change the service from time to time and shall give you notice of any material changes. We may, where we consider it appropriate for you or your protection, suspend, withdraw or restrict use of the service or any part of the service. We will tell you as soon as practicable if we take such action. We reserve the right to vary these terms and conditions.
